Virtual Pro bono Case manager - Community Service




Our virtual pro bono case manager (or V-CM) service in the community refers to a program that provides free, voluntary, and professional assistance to individuals or groups who are in need of support with managing their cases or navigating complex systems (via online, phone and sometimes face-to-face). 

The purpose of our V-CM is to advocate for clients, connect them to appropriate resources and services, and help them navigate through various challenges they may be facing.

The role of V-CM might involve:
  1. assessing clients' needs, developing individualized plans, 
  2. coordinating services, 
  3. monitoring progress, and advocating on behalf of clients to ensure their rights and needs are met
Our V-CM service in the community specialized in the area of social services aims to provide support and guidance to individuals or families facing various social challenges. These challenges may include homelessness, poverty, mental health issues, domestic violence, substance abuse, unemployment, or access to healthcare and social benefits. 

The V-CM role is to assess the client's needs, develop a personalized action plan, and connect them with appropriate resources and services available in the community. Here are some key components of our V-CM service in the area of social services:

1. Client Assessment: The case manager conducts a comprehensive assessment of the client's needs, circumstances, and goals. This includes evaluating their social, emotional, economic, and physical well-being.

2. Action Plan Development: Based on the assessment, the case manager collaborates with the client to develop a customized action plan. This plan outlines specific objectives, steps, and interventions required to address the client's social service needs.

3. Resource Referral: The case manager identifies and connects clients with appropriate community resources and services. This may involve referring them to food banks, shelters, counseling services, job training programs, legal aid, healthcare providers, or other relevant organizations.

4. Advocacy and Support: The case manager serves as an advocate for the client, helping them navigate through complex systems and procedures. They provide guidance, emotional support, and empower clients to become self-sufficient and make informed decisions.

5. Coordination and Collaboration: The case manager collaborates with other social service providers, agencies, and community organizations to ensure a coordinated approach to addressing the client's needs.
