Case management
refers to the overall responsibilities of
Child Safety when providing
ongoing intervention to a
child and their family after the completion of an
investigation and assessment. It is a way of working with the child, family and other agencies to make sure the services provided are:
- coordinated
- integrated
- targeted to meet the goals of the case plan or support plan.
Ongoing intervention is provided when:
- A pregnant woman or young person requires support through a support service case.
- A child is subject to
- an intervention with parental agreement
- a directive or supervision child protection order
- a child protection order granting custody or guardianship to the chief executive.
- A child is subject to a child protection order granting long-term guardianship to a suitable person. In this circumstance, ongoing intervention is provided in a limited capacity. (Refer to Long-term guardianship to a suitable person.)
